Multiple Sclerosis Quality of Life Project's (MSQLP) case management program is crucial to many families in need. We assist clients in a variety of systems such as: navigating the disability process, education about MS and Parkinson's disease (PD), obtaining medical equipment, addressing accessibility issues and finding resources for home modifications, facilitating referrals to various healthcare and rehabilitation specialists, and assistance with enrolling in various state and federal programs (in home support services, financial aid, paratransit, etc.).
Our case management staff work with clients and their families to create comprehensive individualized plans and programs that support client independence. Our case manager meets with clients in their homes and conducts an assessment in 8 areas of needs: Health and Healthcare, Finances, Social Support, Functional Abilities, Activities of Daily Living, Community Services, Home Environment and Perceived Future Needs. MSQLP’s staff then monitors the client’s progress and works with the client to meet identified objectives and solve problems through to resolution. Deep-seated in our case management program, MSQLP coordinates peer-to-peer support to assist in the process of goal completion. In addition, MSQLP's staff conduct quarterly check-in calls with clients to identify any potential new needs and check on progress of client goals.
